All high school band programs in AISD require these forms for  participating in high school band (click the form to download):

• Physical Form

  • Students must take this form to a doctor/ care facility and receive a physical. 

• Medical Authorization Form

  • This form must be signed but does NOT need to be notarized. This allows us to get help for the student should a medical emergency occur.

• Permission to Travel

  • Students may not go on a bus with the band without this form.

• UIL Form

  • This form acknowledges the Texas UIL rules of marching band.

These forms are all REQUIRED to participate in the SHHS Texan Band.
